1. Emergency Situations
For serious or life-threatening conditions—such as chest pain, shortness of breath, severe abdominal pain, sudden trouble walking, sudden vision loss, or uncontrolled bleeding—please call 911 or go to the nearest emergency room immediately.
2. Routine Medical Visits
To schedule a routine medical visit, submit the Request an Appointment form below.
Our office offers affordable rates; we do not accept health insurance.
New patients: Please allow 2–5 business days for scheduling your first appointment.
Established patients: You may request same-day or next-day appointments by logging into your Patient Portal. You can also request appointments for your family members.
Routine lab work: Periodic lab tests may be required for ongoing chronic disease management.
Medication refills (for stable chronic conditions): Please bring your medication bottle or the original prescription to your visit.
3. Immigration Exam Applicants

To initiate your I-693 form or schedule an immigration exam, submit the appointment/payment form below.
Important Notes for Immigration Exam Appointments:
Health insurance is not accepted for immigration exam visits (vaccines may be covered by insurance).
Pricing is fixed and non-negotiable.
Please have your current vaccine records ready for submission.
